If you require a new or first sick note, please contact the surgery at 8am and arrange a GP appointment.

For an extension to a current sick note, please contact the surgery after 9am to put in your request.  Please note a sick line cannot be forward dated so please contact us the day your sick line is due to be renewed or the next working day after where it can be backdated to the date the previous one ran out.
